How to capture the agents and approvers for forms

Hi,

We have to create a dashboard for HR and it should have information like:

1. When and who triggered the form (forms like Job Salary, Medical forms, Termination...)

2. Agents to whom the form is routed for approval (If there are more than one agent, all the agents should be captured)

3. When and who approved the form (which Agent)

Is it good to capture these information in the WF that is triggered when the form is submitted or is there any other way to fetch the above details.
